Terms and Limitations
- Coverage begins upon receipt of a completed and signed contract and when payment is received.
- Membership extends for one year (365 days) for each individual child from the date of receipt.
- The membership fee is non-refundable and non-transferable.
- Patients with traditional dental insurance that Smiley Kids Dental accepts are not eligible.
- This plan is non-refundable and non-cancellable and is only valid for 12 months during regular business hours at Smiley Kids Dental.
- The membership enrollment fee is valid for the 365-day membership period and is subject to change thereafter.
- Smiley Kids Dental has the right to decline re-membership based on prior membership negligence.
- Note: Discount does not apply to fees for general anesthesia.
